To support client referrals, the ideal candidate will be located in or nearby to the Peregian or Noosa areas. You will travel the Sunshine Coast region as required to conduct assessments. This role is a part time position (3 full days/week) on a contract ending June 2022 (with potential for renewal).

This position will commence Mid February 2021. 

What will I be doing as a home support assessor?

· Community based role, visiting older Australian’s in their homes. You will drive throughout your broader region with a rostering team determining the best run to your first client and home from your last client

· Conducting holistic assessments with a focus on wellness and reablement and a client centred approach

· Capturing vital client information to build on the My Aged Care client story

· Developing support plans and identifying unique individual goals

· Connecting people with appropriate services and supports in their community. This may include assistive technology, social support or community care

Need to be/have:

· Knowledge of assessment and goal orientated support planning in aged, community, health or social services

· Previous experience working in aged care, community services, assessment and/or coordination of services

· Confident communication with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach

· The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD. This training will commence mid February 2021.

· Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region

· Located in the Noosa/Peregian areas or within a reasonable distance of these. 

· Comfortable travelling long distances by car

What to bring?

· Relevant qualifications in nursing, allied health, aged care, community services or similar

· Unrestricted drivers licence and own reliable insured vehicle. Salary sacrificed vehicle can be discussed for full time position

· Qualified RAS Assessors are encouraged to apply

· Use of My Aged Care systems and National Screening and Assessment Form (NSAF)

· Registered nurses or allied health professionals encouraged to apply
